READ CAREFULLY BEFORE SIGNING — THIS IS A RELEASE OF LIABILITY

1. ASSUMPTION OF RISK. Ninja warrior training, obstacle use, open gym, competitions, events, parties, and related activities involve inherent risk, including falls, collisions, equipment contact, overexertion, sprains, fractures, and other injury. By participating, I voluntarily assume all risks connected with these activities.

2. RELEASE OF LIABILITY. I release Pinnacle Athletics, its owners, employees, coaches, contractors, volunteers, and facility partners from claims arising from ordinary negligence related to participation, facility use, instruction, events, or equipment use to the fullest extent allowed by Oklahoma law.

3. MEDICAL AUTHORIZATION. I certify that I am physically able to participate and authorize Pinnacle Athletics to seek emergency medical treatment if needed. I understand I am responsible for medical costs related to participation.

PHOTO RELEASE. I allow Pinnacle Athletics to use photos or videos of me or my minor participant for website, social media, marketing, and promotional materials without compensation.

ELECTRONIC SIGNATURE. Typing your full legal name and submitting this form creates an electronic signature with the same effect as a handwritten signature.

04 Acknowledgments

I have read and understand the Assumption of Risks section. I voluntarily accept all risks associated with ninja warrior training at Pinnacle.

I have read and agree to the Release of Liability and Indemnification terms.

I agree to follow all Pinnacle facility rules and safety instructions at all times.

I authorize Pinnacle to seek emergency medical treatment on my behalf (or on behalf of the minor participant) if needed.

OptionalOptional: I grant Pinnacle permission to photograph and video record my participation for use in promotional and social media content.
